Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

A statements with all required codes and information will be provided for each service so that you may be submitt and be paid by your provider for out-of-network services.

• Do I have mental health insurance benefits?
• Does my plan have out of network benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
Cash, checks and all major credit cards accepted for payment.

Cancellation Policy
A 24 hour notice of cancellation is required for all missed appointments. The full cost of the session will be charged when 24 hour notice is not given..
